Chronic Condition Management using Bp Premier — Enhance Efficiency and Patient Outcomes
This course provides practical, bite-sized strategies for managing chronic conditions while implementing planned, proactive, systematic, and person-centred chronic care in your general practice.
You’ll learn how to use Bp Premier to its full potential — improving visibility of chronic condition data and registers, coding and tracking conditions accurately, generating GP Chronic Condition Management Plans (GPCCMPs), managing reviews and referrals efficiently, using Actions and Reminders, running SQL queries, and applying other workflow tools — all designed to enhance patient engagement and improve clinical outcomes.
The modules are purposely short (none longer than 8 minutes) so you can learn specific skills, apply them immediately in your practice, and return later for continual skill building.
